Life and Accidental Death & Dismemberment
Basic Life and AD&D
As an eligible teammate, you receive Basic Life and Accidental Death & Dismemberment (AD&D) insurance in the amount of $50,000. Basic Life and AD&D, through Unum, are provided by TOI at no cost to you.
Supplemental Life and AD&D
You may buy additional Voluntary Life and AD&D coverage at discounted rates. The chart describes the amounts of coverage you can buy for yourself, your spouse, and your child(ren). Find rates in the documents attached on the sidebar.

Evidence of Insurability Form
All Voluntary life and disability elections require an EOI*. The EOI is for questionnaire used by Unum to determine eligibility and approval for the plan. Coverage will begin first of the month following plan approval. You will not be charged for coverage unless you are approved.
Voluntary Short-Term Disability (STD)
Short-Term Disability (STD) provides partial income if you’re temporarily unable to work due to pregnancy or a non-work-related illness or injury. Benefits replace 66.67% of your earnings, up to $2,500 per week. Coverage begins after a 7-day elimination period and can last for up to 51 weeks. In some states (CA, NJ, NY, RI), benefits may be offset by state disability programs.
Voluntary Long-Term Disability (LTD)
Long-Term Disability (LTD) provides partial income if you’re unable to work for a longer period due to a serious illness or injury. Benefits replace 60% of your earnings, up to $15,000 per month. Coverage begins after a 90-day elimination period (or after STD ends) and can continue up to your Social Security Normal Retirement Age. LTD benefits may be reduced by other income you receive, such as Social Security or Workers’ Compensation.
Both STD and LTD are subject to an EOI questionnaire if elected outside of new hire enrollment.
